We provide intermittent, part-time, or extended hours of skilled nursing and home health aide services in the comfort of a patient's residence We provide intermittent, part-time, or extended hours of skilled nursing and home health aide services in the comfort of a patient's residence (the intermittent or part-time services are provided on a visiting basis) We provide Physical Therapy, Speech Therapy, Occupational Therapy, and Medical Social Services on a visiting basis to clients in their places of residence Most of our services are provided directly from one of our full time A Plus Home Health Care, Inc. employees Our services are available 7 days a week, 24 hours a day Our services are coordinated by a Registered Nurse / Therapist managing the care - this includes implementing, revising, and updating care plans, conferencing with physicians, scheduling home visits and supervising health team members
