Morrow, GA, 30260 To Whom It May Concern: Re: Customer#7834555 Green 2008 Mazda 5 October 13, 2012 my 2008 Mazda5 (vin # JM1CR293680313495) was serviced at Hennessy Mazda of Southlake 7261 Jonesboro Rd. in Morrow, GA, 30260. I received a tune-up, oil and filter change, vehicle inspection and emission test. I was assured that my vehicle was operational and in no immediate need of additional services. Furthermore, that there was absolutely no indication that my vehicle was in jeopardy of being inoperable or unsafe to drive. On October 23rd 2012, my vehicle was towed back to the service center at Hennessy Mazda of Southlake 7261 Jonesboro Rd. Morrow, GA, 30260 and was diagnosed as requiring an engine. The engine failure was caused by the loose radiator/reservoir/engine oil caps which were not properly replace following the fluid replacement analysis on Saturday the 13th. Ultimately the job of Hennessy Service Center is to check for problems and this is something that should have been caught. Due to the negligence and reckless service performed on our vehicle, we expect Hennessy Mazda of Southlake to take full responsibility in providing a resolution.
