Community Health Professionals (CHP) is a comprehensive, nonprofit home health and hospice agency serving Northwest and West Central Ohio since 1975. CHP delivers skilled home health services, hospice, nurses, inpatient hospice, private duty, adult day care, physical therapy, occupational therapy, speech therapy and related services.
