LIFE-NWPA Erie provides coordinated care and support for eligible adults age 55 and older who want to remain safely in their homes and communities. LIFE stands for Living Independence For the Elderly and is Pennsylvania's Program of All-Inclusive Care for the Elderly, also known nationally as PACE. Our interdisciplinary team coordinates primary and specialist care, medications, therapy, transportation, home and personal care, nutrition, social services, and activities through one care plan. Participants can also receive services and social support at the LIFE-NWPA adult day health center. We help families and caregivers explore long-term care options and determine whether LIFE-NWPA may be a good fit.
