Healthcare Choices is a Federally Qualified Health Center that offers affodable sliding scale fees for clients that ar eunemplyed, underinsured or are self-pay. HealthCare Choices offer a Patient Portal for patients to have access to their chart, request lab results, and schedule appoinments.
