We offer outstanding care in a home like environment, for seniors who are no longer safe to live independently in their homes . Our Adult Care Home is located in SW Portland, IN WEST HAVEN - SYLVAN COMMUNITY, walking distance to PROVIDENCE ST VINCENT HOSPITAL . This spacious contemporary facility is surrounded by beautiful, manicured gardens where residents can take walks, includes 5 guest suites with their own private baths, shared walked in shower, family room, dining area, kitchen, serene patio. We provide 24 HOUR CARE TO OUR RESIDENTS: assistance/full assistance with activities of daily living ( Eating, Dressing and Grooming, Bathing and Personal Hygiene, Mobility, Toileting, bowel and bladder management and Cognition/Behavior) . We provide END OF LIFE CARE for people transitioning to HOSPICE.
