We value continuity of care and make every effort to ensure you will see your primary care provider for visits. If your D000B0EC is not available, you will meet with one of our other D000B0ECs or pediatric nurse practitioners. In 2010, we adopted the Medical Home model of care. The AAP has defined the Medical Home as accessible, continuous, comprehensive, family-centered, well-coordinated, and compassionate care. Our medical home care coordinator leads our efforts to continually improve our patients' experience and the quality of care we deliver.
