Geriatric care management. We assist families who are concerned about an elderly love one, by putting into place homecare assistance. CNY Elderplanning connects clients with appropriate physicians, home care agencies and community programs. We help set and attain goals based on the client and family. A plan is developed, based on the wishes of the client, which will assist in providing independence. We can coordinate home care, nursing and companion care, meal programs, adult day care, respite, hospice and other services. We can help determine Medicaid eligibility and assist with the application process. If placement in a facility is needed or desired, we help with that process. If adult children live out of town, we can function as a surrogate family to oversee the needs of the older person. As a liaison, we provide objective progress reports and facilitaate communicatin between client, service providers and family
