Norfolk Community Health Care Clinic was established in October of 1999. We are a non-profit 501c3 Federally Qualified Health Care Clinic (FQHC) serving the greater Norfolk and Madison area. The Madison Health Clinic was brought on board in March 2008, and the Dental Clinic opened in September 2009. Our Board of Directors consists of 12 volunteers with expertise in various areas.
