Since 2003 when we visited our first patient in her own home, Evolution Home Physical Therapy, P.C. has continued to excel at providing quality therapy in a compassionate manner. Our team of experienced physical and occupational therapists is waiting to serve you. With skilled therapists that can assist in the recovery from pain, falls, joint replacements, neurological incidents and hospital stays, help is only a phone call away. We visit our patients in their own homes, but as an outpatient practice we can work with our patients when they are no longer home bound, such as after the care of a home health care agency. We specialize in the rehabilitation of older adults, but work with individuals of all ages that may benefit from therapy at home. We strive to provide quality therapy at every visit and find that we exceed most people’s expectations in that we have the time and attention to give to each individual as is often needed and not always provided. Our therapists are located strategically throughout Long Island and eastern Queens in an effort to be there for everyone who may benefit from therapy to feel better and move with greater ease.
