We are pleased to provide a comprehensive custom care plan in addition to a roster of convenient services that exemplify "Integrated Care". We offer the following services to independently owned practitioners in which they may participate in the Medicare Advantage Program: 24-Hour Availability, Appointment Transportation, At-Home Care, Behavioral Health, Community Outreach, Comprehensive Care, Diagnostics and Procedures, Disease Management, FPG Patient Portal, Free Diabetic & COPD Supplies, Hospital/Nursing Home Care, Pharmacy, Seniors Clubs.
