Prenatal Care Coordination is a Medicaid and BadgerCare Plus benefit that helps pregnant women get the support and services. enatal Care Coordination Program Description The Medicaid Prenatal Care Coordination Program helps pregnant women and, when appropriate, their families gain access to medical, social, educational, vocational, and other services related to the pregnancy. Services are available to Medicaid-eligible pregnant women with a high risk for adverse pregnancy outcomes during pregnancy through the first 60 days following delivery. Program components include outreach, an initial assessment, care plan development, ongoing care coordination and monitoring, and health education and nutrition counseling services, as appropriate. The goal of the PNCC benefit is to improve birth outcomes among women who are deemed at high risk for poor birth outcomes. Contact us today !
