At Elder Link Case Management, we provide case management to elders in our community, who may be at risk for going to a nursing home. In order to help them stay at home we will: coordinate in-home services, make referrals to community services, assist to find funding sources, help fill out and submit paperwork, help find housing, provide advocacy for various issues, and act as a source of information. Elder Link accepts private pay and the Elderly Waiver.
