"Connecting our aging population with community services" At Elder Link Case Management, we provide case management to elders in our community, who may be at risk for going to a nursing home. In order to help them stay at home we will: coordinate in-home services, make referrals to community services, assist to find funding sources, help fill out and submit paperwork, help find housing, provide advocacy for various issues, and act as a source of information. We work closely with our consumers to tailor a care plan that will cater to their needs. Mission: "Elder Link Case Management is committed to linking our elder population to information and resources needed to remain safely in their homes through case management services". Elder Link accepts private pay and the Elderly Waiver.
