The quality of life for those we serve is enhanced through palliative and specialized supportive care designed to meet the physical, psychosocial, social and spiritual needs of the patients and caregivers. Hospice service is provided wherever the patient calls home. Our hospice team of physicians, nurses, home health aides, social workers, spiritual care coordinators, bereavement counselor, volunteer coordinator and volunteers work collaboratively to meet individualized patient and family needs. Our team also collaborates with extended care facilities, facilitating the best care possible for our patients who reside in extended care facilities.
