Our Care Coordinators will make a complimentary initial visit to asess your individual needs, and devlop a plan of care. They will also follow up with you regarding the care we are providing to ensure we are meeting your expectations. Their experience in the home care field working closely with doctors, social workers, and therapist gives them valuable insight into other services that might be beneficial to you. if other services are needed beyond our scope of care, they will assist you with obtaining those services.
