Enteric Solutions Corporation (ESC), a charitable organization, develops pharmaceutical formulations for the treatment of diarrhea in developing countries.
Initially, ESC will follow a two-tier approach. In the first two years of operations, regulatory documents, technical and clinical data acquisition will be pursued in the US and possibly in India for an improved formulation of the oral rehydration solution (ORS) containing zinc, a flavoring agent, amylase-resistant starch (RS) and various package presentations. After approval, ESC will scale up manufacturing, likely in India. Sales to UNICEF and/or other interested customers will follow.
Although the conventional glucose-based ORS does not reduce duration or severity of diarrhea it has greatly reduced infant mortality from approximately 5 million to 1.5 million. The current mortality data caused by diarrheal disease reflect in part problems with access, distribution and adoption of the ORS product by the patients. Regardless of these limitations, attempts to improve the ORS composition to create a more effective ORS make sense and are underway.
The rationale for the development is as follows:
1. ORS for treatment of diarrhea relies on enhancement of small intestinal sodium transport and fluid absorption mediated by glucose to correct dehydration.
2. It has been shown that the addition of zinc to ORS can reduce the severity and duration of diarrheal episodes and improve survival. The mechanism of action is multiple and includes inhibition of chloride-dependent fluid secretion, regeneration of intestinal epithelium and enhancement of immune response and clearance of pathogens.
3. Resistant starch added to ORS reduced the duration of diarrhea compared with standard treatment in adults with cholera and in children with acute diarrhea, presumably by generation of short-chain fatty acids in the colon and enhancement of colonic sodium and fluid absorption.
ESC intends to prepare improved, state-of-the-art formulations based on the ORS composition and show superiority in the clinical arena, prior to market introduction. For simplicity, the proposed active components of our formulation are as follows: ORS+Zn+RS.
ESC must partner with like-minded foundations, or individuals interested in financially support improved treatments of diarrhea in developing countries. Provided that financing is secured during Q1/2013, the company will develop and manufacture an improved therapy for diarrhea in 2016.
