BFC CARE OFFERS INDIVIDUAL AND FAMILY PLANS STARTING AT $150 A MONTH Individual plans Bronze plan $150 - Clinical, fiscals, minor injuries, day to day Dr visits and a list of labs, and many more… Silver Plan $225 - Includes all bronze plan services plus: radiology and medication within our formulary - Gold Plan $300 - Includes all bronze plan, silver plan plus: pain management, neurology and rheumatology Our individual plans have no limit of visitations to Dr. Family Plan Family Plan $150 - Includes Dr visits plus any diagnostics or lab work covered under bronze plan Family Plus Plan $300 - Includes Dr visits plus diagnostics or lab work within bronze plan and medication within FBC CARE formulary Silver Family Plan $400 - Includes Dr visits, diagnostics, lab work under the bronze plan and radiology covered within silver plan Family plans are for up to 4 family members, one person goes with no charge second person thereafter have a co pay of $35
