Ambercity Hospice uses a team approach to provide comprehensive care and service to patients and their family right at home, an assisted living facility, long-term care facility, hospital, or independent end-of life care facility. We believe that providing quality care and value added services to each patient are our most important responsibility. Our Hospice patient services are available 24 hours a day, seven day a week, on an intermittent basis and include Prescribed Medication, Medical Equipment, Respite Care and our Professional Health Care team consisting of Physicians (Medical Doctor), Nurses (RN,LVN), Social Service Worker, Home-Health Aides, PT/OT, Chaplain, Bereavement Counselors, and Volunteers.
