Heritage is a multi-building 66 bed personal-care licensed facility under AZDHS and accepts private pay and all ALTCS insurance plans. Heritage specializes in behavioral management for those suffering from Axis-I disorders including: bi-polar, schizophrenia, depressive, anxiety, eating, obsessive-compulsive, panic, and post-traumatic stress disorders. We employ a long-term and recovery based model overseen by Lifespan Behavioral Health. Ours are residents who require personal care assistance with ADLs, medication, eating, etc. yet have been diagnosed with an Axis-I disorder.
