ACS provides services in Orange and Durham Counties in North Carolina for individuals and families to assist them through life transitions. A transition may be required due to an acute health event, an aging parent needing assistance, or indicators of cognitive changes that will require future attention. Our staff provide patient-centered consulting to navigate the health care system to enable individuals and families to make the best choices about local facilities, alternative living situations, in-home care and aging in place. We are knowledgeable about community resources, Medicaid and Medicare and will provide support during physician and emergency room visits and hospital stays. We also provide back-up in order for families to take time off.
