PACE participants are served by a comprehensive team of healthcare professionals including doctors, practitioners, pharmacists, nurses, therapists, chaplains, social workers, dieticians, homecare coordinators, activities coordinators, personal care assistants, transportation professionals, and more. The all-inclusive, nature of care received enables our participants to improve their quality of life. We take care of primary, specialty care, prescriptions, socialization, spiritual care, transportation, audiology, therapy, dental, eye care, and much more. Most seniors would prefer to age in place, in the home they love. PACE services are designed to help seniors remain safely in their homes while also providing respite for family caregiver.
