CCDC provided a volunteer network, education and medical support services as one of 30 community partners in HHS and CDCs model program-Racial and Ethnic Approaches to Community Health (REACH 2010) from September 29,1999 to September 29, 2007. This effort leveraged participation, in REACH U.S. SEA-CEED (Southeastern African American-Center of Excellence in Eliminating Disparities), as one of 40 grantee partners in this model program for communities that demonstrated remarkable health outcomes (blood sugar testing and amputation rates).. CCDC represents a group that can provide "out of the box" community-driven solutions to achieve health equity among underserved racial and ethnic minority populations.
