LIFE-NWPA Mercer provides coordinated health care and support for eligible adults age 55 and older who want to remain safely in their homes and communities. LIFE stands for Living Independence For the Elderly and is Pennsylvania's Program of All-Inclusive Care for the Elderly, also known nationally as PACE. Our interdisciplinary team coordinates medical care, medications, therapy, transportation, home and personal care, nutrition, social services, and activities through one care plan. Participants can also receive services and social support at our Sharon adult day health center. We help older adults, families, and caregivers explore long-term care options and determine whether LIFE-NWPA may be a good fit.
