Our agency provides case management services for people with intellectual and developmental disabilities who are eligible for DDD. Support Coordinators are responsible for utilizing a person-centered planning process to identify outcomes, planning goals, and support needs and develop/maintain the Individualized Service Plan (ISP). In addition, the Support Coordinator also helps the person and his/her family find supports and services that best meet his/her needs and preferences. The Support Coordinator also conducts ongoing monitoring of the provision of services included in the Individualized Service Plan. * Currently, our service area includes the following counties within New Jersey: Essex, Bergen, Morris, Hudson, Union, Passaic, and Middlesex. We are located in the Financial District of Newark, New Jersey.
