Our agency provides case management services for people with intellectual and developmental disabilities who are eligible for DDD. Support Coordinators are responsible for utilizing a person-centered planning process to identify outcomes, planning goals, and support needs and develop/maintain the Individualized Service Plan (ISP).
In addition, the Support Coordinator also helps the person and his/her family find supports and services that best meet his/her needs and preferences. The Support Coordinator also conducts ongoing monitoring of the provision of services included in the Individualized Service Plan.

Throughout the years of servicing individuals with developmental disabilities, we are experienced in working with individuals who are unable to advocate for themselves, being left to feel vulnerable. However, through TaUrt’s Village we are honored to be able to successfully provide a safe domain for individuals who also struggle with gaining independence.
