Our staff strives to provide support to vulnerable populations and the family. Age Smart provides crisis intervention during very challenging times throughout the lifespan. Our geriatric care managers, professional case managers, guardians, assistant care managers and administrative staff work in partnership with the treatment team and family to identify and develop a plan of care. We provide firm and supportive interventions in support of drug and alcohol recovery. We provide support and monitor through the recovery process and transition. We have a geriatric nurse practitioner on staff with an extensive background in the field of elder care and management.
