Horizons Aging Journey was inspired by a personal story — the path my father took through aging in place, living with family, independent living, and ultimately end-of-life care. Along the way, our family realized how critical advocacy is, yet how little clear guidance existed in one place. Even with multiple siblings, some with professional expertise, we often felt lost and unprepared. Out of this experience came a promise: no family should have to walk this journey alone. Horizons Aging Journey is dedicated to equipping families with the knowledge, tools, and resources they need to make informed decisions. From estate planning to caregiving coordination, mental and physical well-being, and end-of-life planning, we offer support every step of the way — so families can focus on what matters most: love, care, and dignity.
