We can also provide care coordinated with the clients Primary Care Physician. When they can't get out, we can come in. We will assess a person's needs, review medications, complete a full body check for injuries/wounds, we will make written recommendations related to our findings and act as a liason for the person and their primary care physician(s). We can also provide skilled nursing care for the client at home and will coordinate care with their primary care physician.
