Evergreen Chalet has 12 bedrooms and all are private. The room, three meals and snacks, assistance with bathing, dressing and grooming, escort service, daily laundry and maid service, medication reminder, activities and two monthly trips to the doctor are all included in one monthly rate of $3,250. The only other additional charge would be for incontinence, if necessary ($250.00). We accept only private pay. Evergreen Chalet is equipped with a security alert system and intercom in each room and has a secured perimeter. We accept ambulatory and non-ambulatory residents, and victims of Alzheimer and related dementia. We create a personalized care plan to meet the special needs of all residents and provide the highest quality of care possible.
